Injectable dermal filler describes the process of injecting a synthetic (man-made) safe-to-use gel-like substance under the skin for the treatment of volume loss and contouring in order to achieve a specific aesthetic effect. The gel is made of Hyaluronic Acid (HA) which naturally occurs in the body and encourages cell hydration, cell nutrition and collagen production. HA draws waters to the area it sits in creating a plump and hydrated surface appearance. Dermal fillers now come in many forms that vary according to their ingredients, how quickly the product dissolves/breaks down, how it works to achieve the desired effect and how long it lasts.

How do treatments work?

Depending on the type of product, dermal fillers work in different ways. Most commonly used in cosmetic practice are those which sit under the skin as volumizers until the injected material biodegrades (the body naturally breaks it down). These are temporary and require repeated injection in order to achieve a continual effect. Permanent dermal fillers also act as tissue volumizers but these do not biodegrade so the effects are long lasting, although results are less likely to be amenable to alteration and side effects such as granulation formation (abnormal tissue overgrowth) can occur. Permanent filler can also be used in scar and acne treatment. Other dermal fillers act as ‘stimulators’ where the stretching of the skin induces an inflammatory response which then stimulates collagen production. These tend to be semi-permanent and are not suitable for those procedures that require superficial (top skin layer) injection such as lips and temporal/galeal areas.

Lip Augmentation

Dermal filler is used to create volume, plumpness, definition and shape to the lips. Depending on the desired result, dermal filler can be injected using various techniques and may require more than one appointment. Numbing cream is used to reduce pain.

Nasolabial Folds

Also known as the nose-to-mouth lines. This treatment smooths the deep lines that run from nose to mouth corners using dermal filler.

Marionette Lines

This treatment is the smoothing of mouth lines that run from the corners of the mouth downwards. The procedure involves injections of dermal filler along the folds to create a smoother plump appearance and to reduce the appearance of lines.

Perioral Sculpting

This is the treatment of deep vertical lines and creases above the lip, sometimes called ‘smoker’s lines’. Dermal filler is injected above the lip to smooth the appearance of lines. Often best results are achieved if used in combination with botulinum toxin injections.

Cheek Augmentation

To create a higher, fuller and more contoured appearance of the cheeks. This treatment also lifts the lower face for a more rejuvenated appearance.

Chin Enhancement

This treatment can reshape the face to create definition and profile harmonisation.

Hands

Hand rejuvenation to reduce signs of ageing and improve loss of plumpness of the back of the hands. This treatment involves multiple injections of small amounts of long-lasting filler across the backs of the hands.

Jawline Sculpting

Creating definition and shaping of the jaw which can also have a slimming effect. This involves dermal filler injections along the jawline and angle of the jaw, and can be combined with chin augmentation to create a complete sculpted appearance.

Tear Trough & Midface Rejuvenation

Dermal filler is used under the eyes and in the midface where fullness is most commonly lost as we age, creating a plump and more youthful appearance, and reducing the appearance of under eye hollowness.

Non-surgical Rhinoplasty

Also known as a 'liquid nose job'. Dermal filler is used to straighten and contour the nasal bridge, whilst also defining the tip of the nose.

Jowl Reduction

Jowls are the sagging of skin around the chin and jaw area caused by volume loss. Dermal filler is injected into the jawline on both sides to restore volume and reduce the appearance of jowls.

Scar Filling

This treatment requires a consultation prior to procedure so the scars and treatment area can be assessed. If suitable, dermal filler will be used to fill the scar and rejuvenate the surface of the scar.

Frequently Asked Questions

Side effects vary depending on the procedure and area being treated. With an experienced highly trained practitioner these risks can be reduced and carried out safely.

 

Possible complications include:

 

● Skin redness; associated with skin needling/manipulation and is temporary

● Pain, burning or stinging at injection site; usually low intensity and resolves within minutes

● Bleeding; most often very minor at injection sites and resolves within seconds. Bleeding can be prolonged if taking concurrent blood-thinning medications, high dose Vitamin E or high dose omega 3 oils, or with underlying bleeding disorders

● Bruising; not uncommon but not usually extensive, can be managed with ice/cold compress

● Swelling; temporary and should resolve within days to short weeks

● Discolouration; can occur when filler is placed too superficially (i.e. incorrectly) or possibly post-inflammation

● Infection; rare but can occur. Can be treated with antibiotic and prevented by cleaning the skin thoroughly prior to injection and good aftercare

● Lumpiness; can occur if too much filler is injected into one area, or repeated injections in the same area too frequently. Can be initially reduced with hot compresses or dissolved with Hyalase

● Granuloma formation; rare, formation of tissue around the injection area after inflammation as the body attempts to block off what it perceives as foreign material. Can be treated with intralesional steroids, antibiotics or may require surgical drainage/excision

● Vascular occlusion/embolization; injection into an artery causing vascular occlusion and damage to the overlying skin and tissues which can lead to necrosis (death) of the tissues. This is an emergency and needs urgent assessment and treatment with Hyalase and potentially further emergency medical/surgical treatment.

Topical numbing cream is used for all dermal filler procedures. We do not use nerve blocks as it is important for us to establish whether there is excessive unexpected pain as this can occasionally be a sign of complications such as vascular occlusion.

You must be 18 or over in order to receive dermal filler treatment. In our younger patients we would always request a full facial consultation first to truly establish whether dermal filler is the best option for you.

Dermal filler cannot be performed if you are pregnant or breastfeeding.

We always recommend you contact the practitioner who performed the procedure if you require reversal of a treatment. However, after a full facial consultation if we feel it is safe to proceed, we would consider offering to dissolve your filler. Often it is necessary to dissolve previously injected filler in order to perform a new procedure, in which case we would offer this.

While we do have to warn you about discomfort, all our high-quality fillers have local anaesthetic in them, and we apply numbing cream to the skin to help. With many of our filler procedures only one injection point is needed so hardly any pain is felt at all!⁠
